NovaCare Nursing delivers advanced clinical nursing and consultancy for people with complex health needs at home, and for providers requiring experienced clinical nursing input. Led by Palantina Hughes, an AHPRA-registered nurse with 17 years of clinical nursing experience, NovaCare Nursing provides skilled assessment, practical care planning, documentation, education, coordination and safe escalation.
Clinical nursing for complex care at home
Services are nurse-led, assessment-based, evidence-informed and delivered within Registered Nurse scope of practice. Care may include clinical assessment, wound and skin integrity care, vascular and high-risk foot support, IV infusions under written medical orders, palliative care, medication-related nursing support, continence and catheter care, respiratory and device support, care planning, documentation, education, referrals and clinical coordination.
NovaCare Nursing does not replace the client's GP, specialist, pharmacist, emergency service, hospital or ambulance service. Medical diagnosis, prescribing and treatment decisions requiring a medical practitioner remain with the appropriate treating clinician.

NDIS Provider SupportIV iron infusions at home
At-home iron infusions may be suitable where the treating medical practitioner has assessed the client, prescribed the medication and provided a valid written drug chart, medication administration order or infusion order. NovaCare Nursing does not prescribe. The Registered Nurse role is to assess, confirm consent, check allergies and the written order, administer the infusion, monitor the client and IV site, document care and escalate any concerns.
Emergency and escalation pathways are used for adverse reactions, clinical deterioration or anaphylaxis. If a serious reaction occurs, the infusion is stopped, emergency care is commenced, 000 is called where required, and the prescribing practitioner is notified.

Care is provided with clinical judgement, clear documentation, dignity, practical risk management and timely escalation. NovaCare Nursing may decline, delay, modify, stop or refer a service if the requested care is outside nursing scope, unsafe, unsupported by required information or clinically inappropriate.

Yes. Supports are delivered and supervised by a Registered Nurse (RN), with governance, education and documentation aligned to NDIS Practice Standards.

Do I need a GP referral?
No referral is required for private bookings. We coordinate with your GP and specialists as needed for safe, continuous care.
Can you help after surgery?
Yes. Post-operative support includes vital-sign monitoring, wound and dressing care, pain control, mobility and falls prevention, and medication management.
Are you insured and AHPRA registered?
Yes. NovaCare Nursing is fully insured for private practice, and care is delivered by an AHPRA-registered nurse.
